Telehealth Informed Consent

This document explains what telehealth is, its benefits and limitations, and your rights as a patient before you use WalkOn RX's telehealth services. By completing an online consultation, you confirm that you have read and understood this consent.
1. Purpose of This Consent
State and federal law require that patients receive certain information before receiving care through telehealth. This page provides that information.
2. Nature of Telehealth Services
Telehealth means the delivery of health care services using electronic communication, including secure messaging and online questionnaires, instead of an in-person visit. Through WalkOn RX, a licensed clinician reviews the health information you submit and communicates with you remotely to evaluate whether treatment is appropriate.
3. Benefits of Telehealth
Telehealth can offer improved access to care, convenience and reduced travel and wait times compared with some in-person visits. It allows a licensed clinician to review your health history and respond without requiring an office visit.
4. Risks and Limitations of Telehealth
Telehealth has limitations compared with an in-person examination. These can include:
• The clinician cannot perform a hands-on physical examination.
• Information you submit may be incomplete, and technical issues could interrupt or delay communication.
• Telehealth may not be appropriate for every condition, and the clinician may determine that an in-person visit, additional testing or referral to another provider is necessary.
• As with any health service, there is a risk that the clinician's assessment based on remote information could differ from an assessment made in person.
5. No Guarantee of Diagnosis or Prescription
Completing a consultation does not guarantee a diagnosis, a treatment recommendation or a prescription. A licensed clinician independently reviews your information and makes all clinical decisions. The clinician may decline to prescribe, may recommend an alternative treatment, or may determine that telehealth is not appropriate for your situation.
6. Your Right to Refuse or Withdraw Consent
Participation in telehealth is voluntary. You may decline to proceed with a telehealth consultation, or withdraw your consent at any time, without affecting your right to future care. Withdrawing consent may mean WalkOn RX cannot continue to provide certain services to you.
7. Alternatives to Telehealth
You may choose to seek care in person from a local provider, urgent care clinic or other health care setting instead of, or in addition to, using WalkOn RX's telehealth services.
8. Emergency Situations
WalkOn RX is not an emergency service and clinicians are not available for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use WalkOn RX for urgent or emergency medical needs. See our full Medical Disclaimer.
9. State Licensing and Provider Qualifications
Clinicians who provide services through WalkOn RX are independently licensed to practice in the state where you are located at the time of your consultation. You can request information about your clinician's identity, licensure and qualifications by contacting support@walkonrx.com.
10. Confidentiality
Information shared during your telehealth consultation is protected in accordance with our HIPAA Notice of Privacy Practices and Privacy Policy. As with any electronic communication, complete security cannot be guaranteed.
11. Patient Responsibilities
You agree to provide accurate and complete information about your health history and current condition, to follow up with your clinician as instructed, and to seek emergency or in-person care when appropriate.
12. Acknowledgment of Consent
By proceeding with a WalkOn RX consultation, you acknowledge that you have read this Telehealth Informed Consent, understand the benefits and limitations of telehealth, and voluntarily consent to receive care through WalkOn RX's telehealth services.
